(12/08) Pennsylvania closes assisted living facility and moves residents
Finding residents at Willow Crest Manor in immediate and serious danger, the Pennsylvania Department of Public Welfare closed the assisted living facility and moved its residents. Two residents had died since Thanksgiving. In mid-November, the Department told the owners that the facility’s six-month probationary license would not be renewed, based on violations (including failure to administer prescribed medications) and criminal assault charges filed against the owner for choking a resident in August.
